What's the easiest way to schedule an appointment?
Appointments can be scheduled in several different ways. During business hours you may call our central phone number 214-575-2663 to schedule an appointment with any our doctors or physical therapists.
Alternatively you may schedule an appointment 24 hours/day, 7 days per week online by clicking here or the book appointment button at the top of this page
The third option is to register for our online Patient Portal. The patient portal will allow access to health records and billing information. Plus patients can schedule appointments, pay bills, and send messages to members of the COR medical team.
Same day appointments are available Monday-Friday.

How can I pay my bill?
Bill payment can be made directly through the Patient Portal.
You may also call our central phone number 214-575-2663 during business hours and our staff will be happy to take payment information over the phone.
Or you may mail or bring your check or credit card information to our main office address at COR, 1120 W. Campbell Road, Suite #109, Richardson, TX 75080. Our office accepts Mastercard, Visa, American Express and Discover as well as checks or cash.

What should I bring with me to my doctor's appointment?
Please bring photo identification and insurance card with you to your appointment. Minors will need to be accompanied by parent or legal guardian.
Please also bring any medical records related to the condition for which you are seeking treatment including operative reports, doctors notes, arthroscopic pictures, X-rays, MRI (including the report), CT scans (including the report).
In addition, please bring all the medications you are currently taking or a detailed list of your medicines including exact dosages.

Which hospitals are you affiliated with?
Our physicians have privileges at Methodist Richardson Medical Center, Baylor Scott & White Medical Center - Plano, Methodist McKinney Hospital, Baylor Surgicare of North Dallas, and Medical City McKinney.
Our office staff will find out for you which hospitals are in and out of your insurance provider's network. Determining which hospital to use is a decision made by you and your physician based on many factors including patient preference, insurance coverage, location of your primary care physician, date of your procedure, and availability of operating room time.
Please let us know if you have a preference so we can do our best to accomodate you.
